- CHRI Intended Use and Privacy Act Statement Required
- Online Application Engagement Agreement (Notarized Statement) Required
- Endorsement License Verification – Required to be completed by at least one primary licensing jurisdiction.
- Verification of Professional License – Official Verification forms issued by other licensing jurisdictions may be accepted if issue date, expiration date and disciplinary history is verified.
- Clinical Neuropsychology Specialization – Application Addendum Required for CN Specialty
- Supplemental Fee Waiver (PDF)– Optional
